Benefits Overview

GROUP MEDICAL*

  • HMO or PPO coverage offered depending on location of employee residence
  • Graduated employee premium contribution based on salary; premiums deducted on a pre-tax basis
  • Coverage offered in three categories: single, double and family

GROUP DENTAL*

  • Plan provides two cleanings/year paid at 100%; fillings paid at 80%, bridge work at 50% after $50 deductible; some ortho coverage – payment based on “reasonable and customary” determination
  • Graduated employee premium contribution based on salary; premiums deducted on a pre-tax basis
  • Coverage offered in three categories: single, double and family

LIFE INSURANCE*  (Basic, Supplemental and Dependent)

  • Standard Coverage:  Twice base salary at no charge to $750,000 (amounts in excess of $500,000 subject to Evidence of Insurability
  • Optional Coverage:  Fee based Supplemental coverage, one and/or two times base salary to $500,000 (amounts in excess of $200,000 subject to Evidence of Insurability) and fee based Dependent coverage on spouse and children in two levels **Must elect supplemental coverage in order to elect dependent coverage at any level.

401(k)  THRIFT*

  • Contribution level -  2-70% of salary, pre tax or after tax, subject to annual IRS limits.
  • Company match - 60% of first 6% of pretax contributions OR 30% of first 6% of aftertax contributions
  •  Withdrawal and loan provisions
  •  Immediate vesting

RETIREMENT  PLAN

  • Company-funded Defined Contribution plan. 
  •  Company contributes 5% of pay (defined as base pay, overtime, bonus and commission)
  •  Immediate vesting

SHORT  TERM and  LONG  TERM  DISABILITY

  • After three month eligibility and on a graduated scale, short term disability pays up to 26 weeks; Long Term Disability may continue the disability payments

FLEXIBLE  SPENDING PLAN

  •  Allows money to be saved on a pre-tax basis for specific medical expenses and/or dependent care
  • Maximum yearly allocation $5,000 per account

TUITION  REIMBURSEMENT

  •  Following six month eligibility, payment of approved education expenses with 50% paid up front, balance upon completion with passing grade

VACATION

  • Employees earn vacation with pay based solely on length of continuous service -- first year of employment, up to 2 weeks earned at .833 days/month; calendar year following completion of 5 years service, 3 weeks earned at 1.25 days/month; calendar year following completion of 15 years service, 4 weeks earned at the rate of 1.66 days/month

 

*New employees are eligible to participate in this benefit on the first day of employment
